Yes it is locked tightly, you think that I didn't try :) This is just a teaser picture, because I'm sure that there is a hatch somewhere, that will lead to underground. The hospital is not in a good shape and that usually means, that something is forgotten to lock.

One might be able to pop the pins out from the hinges without visibly damaging the hatch...
