And also, there is not many ambulance builders that offer 4x4; for the longest time, Ford was the sole provider of a E350 4x4 Platform for ambulance builders which was a good AND bad thing...

The new 4x4 ambulance rigs from AEV are diesel...and you can get them from GMC, Dodge and Ford

They came up as an option for AEV when Ford a few years back first announced they will ONLY sell gas V8 chassis for ambulance and RV builders. There was a huge back lash and I know AMR threatened Ford they would switch to GMC for their fleet if they didn't offer diesel. Ford changed their mind after that, but AEV started to offer GMC and Dodge diesel models in 4x4 after that.

Though the current Ford E450 4x4 they offer for the ambulance is like incredibly weird. There's issues with the head gasket, and the self leveling system that is offered is highly annoying but at least you can turn it off.
