And, interestingly, "Phone service jack" gets 8 returns from Google, and for 7 of them, the "jack" part is actually a proper name (Jack Bauer, jack in the box, etc).

Old 2 prong outlet double t blade on wire mold below an old phone wiring block ive taken these off and installed a new jack cover the old phone cords were permanently wired no jack ,my parents house in 1965 in the early 80s the jack came they called it modular
