This is the results of an inspection my co-inspector (I#2634) and me (I#3145) made on 2010/12/20 of the so-called "Sinteranlage", situated in Duisburg, Germany. Conditions were grey and snowy. This report labels the most striking findings, both positive and negative, with ample photographic evidence. All-in-all, the factory operators were very forthcoming in aiding us with the inspection.

1. Access roads

The access roads were cleared of snow, in accordance to HMDA Norms 14:25.
See next picture for detail.

2. Vertical excrement movement warning regulations

A railway ran overhead. With open bridges, this requires a warning that excrement comes flying from overhead. A form of labeling is not specified. After discussion with colleagues, we deemed an image of a train, with a brown streak coming from above to be an effective warning measure, understood by most. Picture is added as reference.

3. Thorny Bushes

Visible was a fence with sharp points on it. This could be a safety hazard, but there was enough horticultural measures exceeding minimum protective height (1.55m) and density (>12 kg wood / m3 in winter conditions) taken, to prevent the general public from coming near the sharp points. Check.

4. Sports fields

Companies must provide enough opportunities for it's workers to excersize. In this case, this demand was filled in with an entire snowball pitch. However, it was barely used. The company must take more measures that workers actually use the facilities provided to them - if necessary, with force.

5. Warning system

This warning system was found inadequate. The pole was on a standard red colour, even when there was no clear danger at the moment. FEMA regulations require that warning systems must be mounted on a pole that is GREEN in a non-danger situation, and either red/yellow striped (in a situation where the danger is presented by stationary or moving objects, radiaton and/or taxicabs ) or red (in general danger situations). The colour-changing subsystem may be operated manual, trough the application of a brush, or automatic, either by dispensing paint directly or by paintball gun.

A notice will be made in the report.

6. Expansion/Contraction Failure

It appeared to us that snow had not been equally applied to all sides of this piping system. This might be a cause for differentials in temperature expansion which leads to premature failure. However, we were not authorative in this field and set out a question at the institute for pipe contraction.

7. Corrosion

This pipe was corroded to a level that it will pass, but it will have to be repaired before next bi-year inspection.

8. Light

Light conditions in this area were just below norms. However, as it was exceptionally white outside, which might have distorted our readings, we issued no formal warning, just a request to take more thorough readings.

9. Windows

The windows were all open for proper ventilation. We seldomly see that plant owners comply to measures this thoroughly. Very good! However, see next picture.

10. Trip Hazard

The ventilation system (see previous picture) allowed in some snow. While this is not a problem in itself, it buried some cables which then become a trip hazard. Appropriate measures were to either remove copper from said cables or to add a warning sign to them.

11. High contrast

The high contrast between the white skies and the dark exhaust pipes here made inspection difficult. Suggestion for improvement: paint sky blue.

Also, these pipes were not properly labelled.

12. Stair inspection

These stairs were made of standard NEN st:14-1989/5923/12/5 24mm spaced grid steel with galvanization coating, providing enough diameter to perform the buffon needle experiment here. Also, a visual check with my co-inspector proved that part /6 of above regulations was also in order.

13. Safety measures

As a precaution to prevent access to unauthorized personell, these stairs had their first landing removed. However, snow had not been cleared off the removed parts.

14. Motivation

This hook had a sad face, which could lead to a demotivational work environment. Please repaint in pretty colours.

15. Device of unknown function

The factory under-manager present with the inspections could not provide explanation as to the functioning of this device. Our guess was that it was a multi-cupholder, in which case it did was correctly having 4 handholds, so 2 persons can carry it. All too often, one sees multi-cupholders with only 2 or even less handholds.

17. Main office installations

In the main office we found that there was a lot of debris on the floor, again causing trip hazards.

Guard rails were present in accordance with regulation GR 23/B of the AMCH.

18. Elevator shaft

There was also some obstructions present in the elevator shaft on level 3, causing possible malfunction of the elevator. This is clearly a hazardous situation and we ordered the elevator to be used only on levels 1 and 2 until repairs were made.

19. Ventilation

Again, we were amazed at how thorough the ventilation of this plant was. As is visible in this photograph, the plant owner installed a horizontal bar for all openings 3 metres above ground level and higher, so no one could fall out. My co-inspector and me had some discussion on the interpretation of the "above 3 metres" but we settled this dispute with a pass.

20. Markings

There was a clear marking of the center of this pipe by the innovative use of icicles.

21. Labeling procedures

An individual had been shot here recently. This was clearly labeled on the floor as by police regulation 10-50.

22. Smoke test

We performed a smoke test by breathing on a torch, but smoke didn't get to dangerous levels. Proceed.

23. Family pictures

Inspection of worker's lockers found that most had used the upper area - specifically designated for that purpose - for the application of family photos.

24. Snow buildup

Snow on these stairs (which do fall under NEN st:14-1989/5923/12 and following sections) was not properly cleared, leading to snow buildup above standard levels. This is not a problem as this is not an emergency stairwell.

25. Correct labelling

The fence was left open, so as to allow Parents (Eltern) to search their kids, if they went missing on or near the factory grounds.


This factory is a prime example of good follow-up on regulations making this a safe and good place for everyone to work. Not everything can be good and our main negative findings were a light build-up of clutter creating trip hazards in some places.
It was clear that a lot of work was done to boost morale by creating an inspiring environment to work in.
We hereby award this factory a "pass" for the 2010/11 winter inspection.

Did you perform a risk assessment before posting this thread?
