the new york subways run on a "third rail" which is very high voltage, and one mis-step on it and you are as good as dead... the subways in new york city also come above ground when going from manhatten to brooklyn...the subway riding deaths are not that uncommon, but then again so are the ones from people falling/being pushed on the tracks...

Anyway..
In New York City, as well as here in Toronto, the subway trains work on a traction power rail (aka, 3rd rail). This rail carries +600VDC. The closest regular rail is connected to the power supply at the 0VDC potiential - ground level essentially.
I don't know about in NYC, but in Toronto, they are going to be converting that DC to AC in the future because all the new subway trains here in the TTC use AC motors. Right now, they have inverters on the cars, but AC power distribution would be alot more efficient. Only problem here is that AC does a MUCH better job killing you.
If you touch the 600VDC traction rail, it /probably/ wouldn't kill you unless you had contact with it for more than a second or two. The voltage is great enough that it would cause your muscles to spasm enough to probably throw you off of the rail. AC on the other hand, would cause your muscles to seize, making you unable to move off the rail, and well... BBQ person...
I don't know of anything like that happening in the TTC, although one would only assume that when somebody jumps, they probably hit the traction rail, but hopefully they have already expired at this point, cause it wouldn't be too pleasent.
You might be thinking of streetcars. They use an overhead wire with 600VDC, and in some cities, streetcars run underground for a fair distance (such as Toronto's LRT).

This doesn't mean you won't get fried, it just makes it a bit safer for you to navigate- you can still come in contact with it from the side, and those pieces of wood (not sure about the plastics, they're newer) are old, and if you're heavy they might just snap under your foot if you step on them.
It's very likely you just climb out between the cars. I guess how difficult it is to climb from there depends on which train you're climbing on.
Oh, and how is it safer to train surf inside a tunnel than outside?
By the way, there are people who "ride" on the side of the train. I've done this before and hurt myself pretty badly when I jumped off. There's a ledge on the doors of the train that you can stand on, and you can hold onto the sides of the door. It's pretty stupid.
Then there is bus surfing, or whatever it's called. You stand on the bus at the rear bumper (I think).
It's a crazy city. This stuff isn't at all that amazing. You see something abnormal everyday.

Subway deaths here are pretty common too- I've started a thread on the Ltv board to keep track of them (registration required: http://mtska.com/MTBomb/viewtopic.php?TopicID=718 )
It's easy enough to climb up between cars, or hang onto the side on that little lip by the door. On the older cars, you can pry the doors open a bit too.
While NYC subway is 3rd rail (no plans to go to ac propulsion that I know of- I imagine that would be a bit of a task. they're having plenty of trouble just putting CBTC on the L line as it is.), amtrak through town is all overhead wires. In fact, the tunnels to and from penn station contain both overhead power and third rail for LIRR. Those tubes are a deathtrap, and I'm honestly happy that they have secured them as well as they have.
