I think it's more general, "If it is very heavy, look for help!" Don't get hurt on the job and all that jazz. But the image suggests the guy needs help with his wife, presumably on their honeymoon. Disturbing, I agree. I took it as a safety message couched in a bizarre masculinity challenge that runs counter to the safety message. Ah, Texas.
