Is this done when taking the actual shot or afterwards in Photoshop? How do you get it to follow the road like that?

It was done in photoshop using lens blur, simulating a tilt/shift lens

Thank you for sharing, so if Photoshop is just simulating the effect, can this be done while taking the photo? I'll have to Google for more examples, this is a pretty cool effect.

It can be done while shooting if you have a tilt/shift lens. My camera has a fixed lens, so it was done in photoshop.

There's a cheap version of a tilt&shift lens called lensbabies. Have a look.
