you lay on your side and put one arm through first wit your shoulders rotated so ones lined up in front of the other and stick your head inside the elbow of your forward arm, then put the other arm in front of your stomach and use it to push yourself along, if you have to you can angle your hips a little bit to minimize there profile

1: oxygen: there are meters available. A very cheap trick (if you have the time) is the following: have a little dish with water and a lit candle in the middle. Put a glass jar over the candle. The candle will go out and suck up water because all the oxygen is bound in CO2. The percentage of water relative to the full jar is the oxygen percentage, so make a marking first. Normal is 20-21%, I think a human needs at least 7 or 8%. However, I don't think your problem is lack of oxygen but buildup of other gases, for example methane, if a lot of rotting takes place.

2. The smallest I remember going trough is in a fort near Liege: a tunnel I think 30x50 cm 1'x1'8" about a metre (3') long, going in a sort of downward bend (so you could only crawl trough head-first, face-up, not face-down). However, after that metre, it was way bigger, maybe 1.5 or 2 metre high.

Well I have slipped through some pretty tight spots but I guess the smallest would have had to be during a caving trip.

Me already half way through the hole. You have to exhale all air in your lungs and slowly squirm your way through. You are not able to breathe while you are going through this hole. Thankfully it was only 5 or 6 feet long before the passageway opened up enough for you to sit with your head down but you can breathe at least.
