Is it just localized around the downtown core, or does it happen outside at places like Larkspur and Sausalito as well?

Not so much in those places (hills and affluence tend to minimize) but it happens all around. In Napa at least one homeless person a year has a tunnel into the river banks and drowns when the water levels rise

In Illinois, I haven't seen too many needles to be really worried about. Only came across them a few times on the way to abandonments, usually under bridges and the like, I haven't seen any needles in an actual location I was exploring, the worst was a lighter and cigarettes.

I've seen it at its worse near the Hayes & Larkin split on the backside of the bill graham civic auditorium. Literally blocks from city hall, the most disconnected people on the planet, san francisco politicians.

That stretch back there is baaaaad. I once parked on the Larkin side during a show at war memorial and deeply regretted it when it when returning to my car
